The GIM clinical pharmacists work collaboratively with the physician group to manage chronic disease states such as diabetes, hypertension, lipids, and smoking cessations. The pharmacists also complete comprehensive medication reviews to ensure patients are taking the most effective medication regimens, address drug related problems, and medication side effects. The pharmacists can be consulted by the patients of GIM with any medication questions or concerns
